Benefit Overview

The ADN Dental Plan helps employees and their families maintain good oral health through affordable coverage for preventive, restorative, and major dental services. Coverage is provided through the ADN Dental Network and DenteMax PPO Networks, offering flexibility and savings when you visit participating dentists.

Available to full-time employees regularly scheduled to work 30 or more hours per week. Coverage is also available for eligible spouses and dependent children up to the age limit stated in the plan documents.

Key Details

The dental plan includes coverage for routine exams, cleanings, fillings, crowns, and orthodontic care for dependents. There are no deductibles, waiting periods, or missing tooth clauses, making it easy to access care as soon as you’re enrolled.

Full Mouth Series or Panoramic X-Rays	Once per 60 months
Composite and Amalgam fillings	Once per tooth surface per 24 months
Onlays and Crowns	Once per permanent tooth per 60 months
Class III Major Services – 80%
Complete and Partial Removable Dentures	Once per arch per 60 months
Fixed Partial Dentures (Bridges)	Once per area per 60 months
Endosteal Implants	Once per permanent tooth per 60 months
Class IV Orthodontic Services – 50%
Limited and Interceptive Treatment	Removable and Fixed Appliance Therapy, up to age 19
Comprehensive Treatment	Fixed Appliance Therapy, up to age 19
Not Covered	Sealants, Eposteal & Transosteal Implants, TMJ/TMD Treatment, Cosmetic Treatment

How the Plan Works

When you visit an in-network dentist, your out-of-pocket costs are lower, and claims are handled automatically. Preventive services are covered at 80%, which means you’ll pay little to nothing for checkups and cleanings. Major services like crowns or dentures are also covered at 80%, while orthodontic treatment for dependents is covered at 50% up to the lifetime maximum.
